2. Eligibility: Sweepstakes open to legal residents of the 50 United States or the District of Columbia, who are 18 years of age or older. Employees of ComSonics® Inc. and their subsidiaries, divisions, licensees and advertising and promotion agencies (collectively "Sweepstakes Entities") and all of the above's immediate family members (parent, child, sibling, and spouse of each) and those living in the household of same are ineligible. Sweepstakes subject to all applicable federal, state, and local laws and regulations. Void outside the 50 United States/D.C. and where prohibited by law.
   
   
3. Sponsor/Administrator: The Sponsor of the Sweepstakes is ComSonics® Inc. located at 1350 Port Republic Rd, Harrisonburg, VA 22801 (hereinafter referred to as “ComSonics”)
   
   
4. Sweepstakes Period: Sweepstakes starts October 28, 2009 at 12:00:01 a.m. EST and ends March 31, 2010 at 5:00 p.m. EST ("Promotional Period"). During the Promotional Period there are two ways to enter: 1) by filling out and turning in an official entry form at the ComSonics booth at the SCTE Cable-Tec Expo 2009 in Denver, or 2) by calling ComSonics Sales at 800-336-9681. Ask to be entered into the ComSonics® Dream Island Vacation Sweepstakes. No other method of entry will be accepted. Limit one (1) entry per person/email address, regardless of method of entry.
   
   SCTE Cable-Tec Expo Entry Method: Go to the ComSonics booth #5081 during the show exhibit hours between Oct 28th through Oct 30th 2009 at the SCTE Cable-Tec Expo in Denver, and fill out/submit the official entry form obtained from select sales representatives at the booth. Entry form requires you to enter your name, company, position, company address, date of birth, email address and phone number. Entrants must have a valid email address at the time of entry and must complete all required information for entry to be valid.
   
   Call-in Entry Method: Call ComSonics at 800-336-9681 and ask for Sales Support. Once connected to a sales support representative, request to be entered into the ComSonics® Dream Island Vacation Sweepstakes. A representative will require you to provide the following information: your name, company, position, company address, date of birth, email address and phone number. The representative will record your information on an official entry form and submit into sweepstakes entry. Call-in entries must be made no later than 3/31/2010 before 5:00 PM EST to be eligible for the drawing.
   
   Multiple entries per person, including multiple entries by the same entrant using multiple/different email addresses, identities, registrations or any other methods, will invalidate ALL entries submitted by that entrant for the entire Promotional Period. Entries become the property of Sponsor and none will be returned or acknowledged, except as provided herein.
   
   
5. Drawing: A random drawing will be held and Winner will be identified on or before April 5, 2010 from among all eligible entries received during the Promotional Period, to determine one (1) Prize Winner by the Sweepstakes Administrator, whose decisions on all matters pertaining to the Sweepstakes are final. Potential Winner will be notified by phone, email or mail. Potential Winner may be required to sign an Affidavit of Eligibility (which affirms that he/she has complied with these rules) as well as a Liability Release, Form W-9 and a publicity release (where permitted by law), all of which, if issued, must be completed, signed, notarized and returned within ten (10) business days from date of issuance or the prize will be forfeited and may be awarded to an alternate Winner. The guest(s) may also be required to complete, sign, and notarize a liability release that potential Winner must return to Sponsor within ten (10) business days. If the potential Winner fails to provide his/her Social Security Number for such prize within ten (10) business days after request is made for the same, said Winner will be disqualified and an alternate Winner will be selected. If the prize is declined, the prize or prize notification is returned as undeliverable, or in the event of noncompliance with these Official Rules and Conditions, the prize will be forfeited and may be awarded to an alternate Winner.
   
   
6. Prize, Odds and Approximate Retail Value (ARV): One (1) Prize: The Prize consists of a Dream Island Vacation for two (2) which shall include: a gift certificate to Sandals® Resorts valued at $5,000 and redeemable for both land and airfares between the United States and the selected resort Sandals® location. Prize Winner and his/her guest must book travel at least ninety (90) days in advance. All travel must be completed by September 30, 2011 or prize is forfeited. Prize Winner and his/her guests must travel on the same itinerary. Sponsor will not replace any lost or stolen tickets, travel vouchers, certificates or gift cards. Once booked, no change, extension or substitution of trip dates is permitted, except by Sponsor at its sole discretion. Tickets are non-refundable and not redeemable for cash or credit. Tickets may not be transferred or sold to a third party. Prize Winner and guests are responsible for obtaining all necessary and required identification and/or travel documentation required. Travel arrangements must be made through Sandals® All travel expenses not described herein as being awarded, including, but not limited to, incidental hotel expenses, meals, ground transportation, gratuities, and souvenirs, are the sole responsibility of the Prize Winner. Any and all costs associated with reaching the departure city will be the Prize Winner's sole responsibility. Sponsor is not responsible for cancelled, delayed, suspended or rescheduled events (including airline flights beyond its control). If for any reason any portion of the Prize does not occur, such portion will be forfeited with no compensation being paid in exchange thereof. Odds of winning depend on the number of eligible entries received during the Promotional Period.
   
   
7. General Prize Restrictions/Total ARV: No cash redemption of the prize. No transfer of the prize. Prize may not be substituted, except by Sponsor for a prize of equal or greater value or at Sponsor's sole discretion. Prize may be different from that pictured in advertising. Taxes on prizes are the sole responsibility of the Prize Winner. Acceptance of a prize constitutes permission for the Sponsor and its designees to use Prize Winner's name and likeness for advertising, promotional and other purposes, on a worldwide basis, in any and all media now and hereafter known, without notice or additional compensation unless prohibited by law. Participation and acceptance of any prize by guests, constitutes guest's permission for the Sponsor to use the guest's name, photograph, likeness, statements, voice and address (city and state) on a worldwide basis without notice or further compensation except where prohibited by law. The Prize Winner acknowledges that neither Sweepstakes Entities or their directors, employees or agents have made nor are in any manner responsible or liable for any warranty, representation, or guarantee, express or implied, in fact or in law, relative to any prize, including but not limited to its quality, mechanical condition or fitness for a particular purpose. Any and all warranties and/or guarantees on a prize (if any) are subject to the respective manufacturers' terms thereof, and Prize Winner agrees to look solely to such manufacturers for any such warranty and/or guarantee. The total prize ARV is $5,000.00.
   
   
8. Limitation of Liability: The Sponsor, Sweepstakes Entities, Administrator, any Internet access providers, and their respective parents, affiliates, divisions, licensees, subsidiaries, divisions, and advertising and promotion agencies, and each of the foregoing entities' respective employees, officers, directors, shareholders and agents (the "Released Parties") are not responsible for incorrect or inaccurate transfer of entry information, human error, technical malfunction, lost/delayed data transmissions, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software or any combination thereof, inability to access web sites, damage to a user's computer system (hardware and/or software) due to participation in this Sweepstakes or any other problem or error that may occur. All incomplete entries will be disqualified. By entering, participants agree to release and hold harmless the Released Parties from and against any and all claims, actions and/or liability for injuries, loss or damage of any kind, to person or property, arising from or in connection with participation in and/or entry into this Sweepstakes, participation in any Sweepstakes-related activity, any service provided by parties other than the Sponsor in connection with the prize awarded, or participation in any prize-related activity or any travel related thereto (including, without limitation, claims, costs, injuries, losses or damages related to personal injuries, death, damage to, loss or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light). Entry materials that have been tampered with or altered are void. Sponsor is not responsible for any change of email, mailing address, or telephone number of entrants. The use of any automated launching or entry software or any other mechanical or electronic means that permits the participant to automatically enter repeatedly is prohibited. Any person who uses or attempts to use such means to enter the Sweepstakes will be disqualified from participation in the Sweepstakes and will be ineligible to receive a prize. If for any reason this Sweepstakes is not capable of running as planned, or if this Sweepstakes or any web site associated therewith (or any portion thereof) becomes corrupted or does not allow the proper playing of this Sweepstakes and processing of entries per these rules, or if infection by computer virus, bugs, tampering, unauthorized intervention, affect the administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Sponsor reserves the right, at its sole discretion, to disqualify any individual implicated in such action, and/or to cancel, terminate, modify or suspend this Sweepstakes or any portion thereof. In the event this Sweepstakes is cancelled, Sponsor will conduct a random drawing to award the prize from among all eligible, non-suspect, entries received prior to the time of the action or event warranting such cancellation.
   
   
9. Disputes: In the event of a dispute as to who submitted an online entry, the entry will be deemed submitted by the authorized account holder of the email address submitted at the time of entry. "Authorized Account Holder" is defined as the person assigned to an email address by an Internet access provider, online service provider or other organization responsible for assigning email addresses for the domain associated with the email address in question. Any attempt by an entrant or any other individual to deliberately damage any web site or undermine the legitimate operation of the Sweepstakes is a violation of criminal and civil laws and should such an attempt be made, the Sponsor(s) reserve the right to seek damages and other remedies from any such person to the fullest extent permitted by law.
   
   
10. Winners List: To obtain a Winners list send a self addressed stamped envelope to: ComSonics® Dream Island Vacation Sweepstakes, P.O. Box 1106, Harrisonburg VA 22801. Requests must be received by June 14, 2010.
    
    
11. Right to Change: ComSonics has the right to change or alter the rules of the ComSonics® Dream Island Vacation Sweepstakes at the sole discretion of ComSonics. All changes to the contest will be reflected in the Official Rules found at www.comsonics.com/makeover. ComSonics is not obligated or required to notify any entrants if changes occur.
